Summit Registration: The fee includes continental breakfast, lunch, refreshment breaks and all summit materials and programs. Space is limited. Registration closes on October 25.
One registration form is needed for each individual. Single registration rates apply to all registrants. Unfortunately, we cannot offer group rates.

Gala Ticket
Contribution includes cocktail reception, dinner, and evening program honoring John K. Iglehart, Health Affairs’ Founding Editor. A portion of the gala ticket is tax deductible.
Confirmations: Registration confirmations will be sent to the email address provided on the registration form. Please call (202) 719-8061 if you do not receive confirmation within two weeks after registering.
Cancellations: Cancellations must be received in writing before Monday, October 15, 2007 in order to receive a full refund, less $100 processing fee.
Modifications/Substitutions: Substitutions will be accepted only once per registrant and must include written notification from the original registrant and a completed registration form for the substitute participant. |
